EXTREME SPORTS konzentriert sich auf Actionsport und adrenalingeladene Wettkämpfe, die in ganz Deutschland ausgestrahlt werden. Der Sender zeigt Disziplinen wie Skateboarding, Snowboarding, Motocross, Surfen und Klettern und berichtet dabei häufig über Events internationaler Serien sowie lokale deutsche Wettbewerbe. Dokumentationen über Athleten und Aufnahmen hinter den Kulissen von Trainingsroutinen oder Wettkampfvorbereitungen sind ein fester Bestandteil des Programms. Musikvideos und Kurzfilme aus der Extreme-Sports-Kultur laufen zwischen den längeren Sendungen. Die Programmgestaltung verzichtet auf kommentarlastige Studioproduktionen und lässt stattdessen die Sportaufnahmen für sich sprechen.

Zuschauer in Deutschland können den Sender über die gängigen Kabel- und Satellitenanbieter empfangen. Ein separater Online-Streamingdienst bietet Live-Übertragungen von Events sowie eine On-Demand-Bibliothek mit vergangenen Wettkämpfen und Dokumentationsbeiträgen.
