Mezzo Live HD diffuse en haute définition des concerts de musique classique, de jazz et de musiques du monde. La chaîne privilégie les enregistrements live dans les grandes salles et festivals d’Europe et d’ailleurs, incluant productions d’opéra, concerts orchestraux et récitals en solo. Au Portugal, les téléspectateurs peuvent accéder à cette programmation via certains abonnements aux câble et satellite proposant des chaînes culturelles internationales. Le contenu est majoritairement constitué de performances musicales sans interruption, avec des documentaires occasionnels sur des compositeurs ou des techniques instrumentales.
Les abonnés peuvent également regarder la chaîne en direct sur la plateforme en ligne de leur opérateur, selon le niveau d’abonnement. Le guide électronique des programmes liste des blocs de concerts quotidiens, souvent regroupés par genre ou artiste vedette, sans pause publicitaire entre les morceaux. Aucun contenu original en portugais n’est produit pour ce flux ; tous les programmes proviennent de la bibliothèque de la chaîne française Mezzo, qui sélectionne des enregistrements haute définition.
