Mezzo Live HD überträgt Konzerte aus Klassik, Jazz und Weltmusik in hochauflösender Qualität. Der Schwerpunkt liegt auf Live-Mitschnitten aus bedeutenden Konzertsälen und Festivals in Europa und darüber hinaus, darunter Opernproduktionen, Orchesteraufführungen und Solorezitale. In Portugal können Zuschauer dieses Programm über ausgewählte Kabel- und Satellitenpakete empfangen, die internationale Kulturkanäle führen. Die Inhalte bestehen überwiegend aus ununterbrochenen Musikdarbietungen, ergänzt durch gelegentliche Dokumentationen über Komponisten oder Instrumentaltechniken.

Abonnenten können den Sender zudem live über die Online-Plattform des Anbieters streamen, was jedoch vom jeweiligen Abo-Modell abhängt. Der elektronische Programmführer listet tägliche Konzertblöcke, oft nach Genre oder Künstler sortiert, ohne Werbeunterbrechungen zwischen den Stücken.
